    Login Patient Portal >>

The practice has implemented a new Patient Portal: Uw Zorg Online. Through this portal you will be able to access your medical records, order repeat prescriptions, start E-consultations and book appointments with the GP. You can register through clicking on the button above.

You will be required to create an account, upon which a request will be send to the GP to link your records to the Portal account. Before we establish the link, we will contact you to verify the request. There is also an app of the portal available at the App Store and Google play store. In the app you are able to change the language of the portal.

Note that making appointments through the portal is only meant for making appointments regarding non emergent care. This also applies for E-consultations. If you need to see the GP urgently or if you question the urgency please call the GP practice, you and the assistant can evaluate this together.

You are not required to register for the patient portal. If you are not willing or able to use it, you can always contact the practice by phone or through E-mail.
